Ok, is the notification being overridden at all? I.e. can you see your custom notifications from BNFw or does it still show the default notification from WordPress still?

How are your creating new users? Is it via the WP Admin, via the standard WP user registration form, or via a front-end form supplied by a plugin or theme?

Can you try:

[email_user_firstname]
[email_user_lastname]
[email_user_nickname]
