Hello,
From what i can see on your store you have a theme issue. I noticed your site is loading an older version of jQuery – jquery-2.0.3.min.js
Your theme is the cause. Make sure your theme is up to date according to WordPress standards, that means including the recommended if not latest versions of javascript libraries like jquery.
You can change to the default 2010 theme from wordpress (Flush theme cache after change) to be sure its your theme.

@akiyo your store seems to work correctly when adding a product to the cart from the product page
Thank you, Mihai for checking into it! I think the problem happens sometimes after people fill out the form and click “purchase” and it clears the form so they need to fill it out again. I also have a couple other problems during checkout like shipping to international and processing into the PayPal site. Not sure if I should start a new topic or keep asking questions here.
