Setting Cookie from Outside WordPress – more details?

The original post about this didn’t cover the most important bits.

1) which file / before/after which function to add the cookie snippet?
2) Exactly what format the two cookie names are? I get something like ‘wp_user_#name hash#_#url#’ ?
I simply need to be able to copy the names over so that any cookies set in headers are right!
