separate page contact form with image uploader

    Hi,

    I’m looking to have a separate page linked through the main page in which I can allow users to upload a picture and then answer 5-6 questions of my choosing about said picture so that I can sift through the entires and pick the ones I wish to post.

    Do you know if there is a plugin that would accommodate something like that?

    cforms by delicious days has a file upload option field but you have to edit the options to allow image files then just build a form that asks your questions
