Separate contact form for each post?

Hi everyone,

How can I have a separate contact form for each post?
Say if the post is on example.com/post-1
I want a contact form at example.com/post-1/form
It’s actually going to be a booking form so I want some information about the post to be loaded in the /form page…

As long as I get the URLs to work correctly I think I can manage the rest of it. So in a nutshell if the post is on example.com/post-1 I want to be able to show some HTML in example.com/post-1/form and be able to retrieve the post id.

I searched the net and this forum for something similar but I couldn’t find any. I also tried some contact form plugins but they weren’t really what I’m looking for.

Please share your ideas.
