Send email after plan is chosen in Pricing Template

    I have configured the page of pricing table (official template of Orbit Fox). What is the simplest way (without knowledges of programming) to add a some function to the button of the plan? For example opening a form which a client fill with his contact information (phone number, email, name). And I will receive an email notification about it, as well as what plan the client has chosen?
    Thank you.
    P.S. Email sending is working.
