Searching using checkbox forms

    Hello,
    please visit the site http://www.simplymarry.com/
    you can see a form on home page of this site, which enables you to search or find a match using the checkboxes, how can we implement this feature in wordpress? is there any plugin? or do we need to make changes in code? or what is the way to create a replica of the same form?
