If you install Advanced Ads Pro, you will find a checkbox “display only once” in the edit menu of every single ad unit to avoid such an issue.
If you are not using Advanced Ads Pro, I would suggest as a workaround adding more than three ads to the group to reduce the probability that the same ad is showing up more than one time.

Bit the bullet and purchased the Pro version as the Grid option gave me what I wanted. However there are no containers around each individual ad which I found surprising. This would very handy for targeting CSS.
The “display only once” option is against the whole page, not a particular ad group or placement. We don’t mind of the ad is displayed in other places on the page.
In any case, the grid ad group sorted the problem out and we will work out a work around about formatting without the containers.
(I know you can put a container on each advert but that’s not very easy for admin staff to do).
There certainly seems to be problems, from our point of view, with the settings for each element. It appears that all the targeting options are at the ad level when it would be very handy to have some of these at the placement or ad group level (ie. home page banner placement). Luckily we can get around this by code.
