registration form – checkbox for previous attendees

I’ve made a simple registration form where you can sign up to yoga classes, and would like the option to check off a box if you’ve signed up to the class before. I want it to grey out every box except for the “name” field, so that they only have to submit their names if they’ve attended the class previously. Is there any way to do this?
