 * Hi,
 * I am running a competition to gauge the success of a new product. I want people
   to carryout the following:
    - Click on a link on my site to take a poll on the product
    - Once they’ve answered they must enter their details, including a barcode number
      from the product (to prove purchase)
    - I want to capture their response on my DB so that I can draw the winner to
      see who wins an ipad
    - They must have tried the product to take the poll.
    - Any idea what plugin combo I could do to make this simple system work?
