Red letter warning – script not working

Hi,

My web page will not process the form on this web page when it comes to the attachments that I have tried to set up:

I have changed the permissions on the index.php file located within the cache to 776 but still get the same error? I have also checked the box “enable .php sessions”

Even the text for the file types is showing below the form field as you will see?

Seems this script is full of bugs and I am considering ditching it to be honest.
