 * I made a beautiful form for donation to a not-for-profit and it disappeared! 
   It literally VANISHED! I bought the plugin through THEIR website and was given
   no way to enter a license number. I am really furious since it took me a lot 
   of time to construct the perfect form AND NOW IT IS GONE!
