    Don’t worry, your plugin file on your website isn’t modified, as I can see from the screenshot you provided the plugin author changed the plugin read-me file for this particular string Tested up to: 4.5 without changing the version number of the plugin Stable Tag, so when Wordfence check for modified files it simply compare your current files with those on the repository of the same version, that’s why it found this difference.

    Ideally, when developers do changes to their plugins files on WordPress repository, they need to change the plugin version to avoid such problem, by this way Wordfence will just ask you to update your plugin to the latest stable version, check this part of Wordfence documentation about this topic.

    But the original file says 4.5 and the file on wordpress.org says 4.5 , why would it modify to 4.0 ? I guess that part isn’t clear to me.

    The file on your website isn’t modified, this is a false positive result, please check this screenshot for more details.

    You can send the plugin author a friendly note about this issue, he/she will understand exactly this kind if issues.
