Radio Buttons – Multiple Other Fields Appear

    When the “other” option is selected in a group of radio buttons a text field appears below – this is ok. However, when another radio button is selected and the “other” option is selected again, an additional “other” text field appears. This continues indefinitely. Any thoughts? Bug?
