In the form preview I can select the radio buttons that I have configured, however on the page where I have inserted the form I click on the radio buttons and they are not activated. Additionally, the selection of the calendar date comes out differently in the background color and text format in the preview vs a in the page where the form is embedded. Please let me know how to fix this.

I hope you are doing well.

Can you please go to WordPress > Settings and double check if the website is configured to HTTPS?

The website does load on HTTPS but the URLs are from HTTP making the browser to block the style request:

You can try a plugin like https://wordpress.org/plugins/really-simple-ssl/ to fix mixed contents.
