Hi,

there is the same issue like topic https://wordpress.org/support/topic/radio-button-label-is-been-added-in-the-value-part/

in your add_filter(‘woocommerce_form_field_radio’, array($this, ‘custom_field’), 10, 4); you put the label value in the input value.

The issue on the topic has been already fixed a time ago.

Please confirm you have the latest version of the plugin and that your overall website is up to date (plugins, theme, WP core)

Also, debug as following if you haven’t done yet:
- Deactivate plugins one by one checking each time.
- Switch to Storefront theme and test
- Disable any custom scripts you might have
