 * I have 4 items from the drop-down menu that can be selected:
 * is it possible to activate the same NAME and SURNAME fields by selecting both
   PRIVATE and PROFESSIONAL, without having to replicate them again within your 
   plugin?

 * We are working on adding this enhancement and as soon as we release a new version
   of the plugin with improved conditional logic it will be possible to select multiple
   values from the last value field. I will notify you as soon as the update is 
   available for download.

   this possibility. You can perform an update of the plugin and enjoy the new conditional
   logic, which offers great possibilities.
