%QUESTION_ANSWER_X% returning 1st question only

    %QUESTION_ANSWER_X% always returns the first question and users answer to the first question, no matter what number is substituted for X.
    For example:
    %QUESTION_ANSWER_3% will return a copy of the 1st question and the user’s answer to the first question, not the 3rd.

    The issue can’t be replicated on our end. Can you please share step-by-step instructions to generate the issue or a screenshot/ screencast if possible so we can check and help you to fix the issue.
