putting items on separate line

    Hi all, I’m trying to figure out how to create checkbox items on a separate line. If I use multiple text box items it puts them all next to each other, making it quite cluttered looking. Instead I’d like each check box item on a new line. I tried adding <br> between each item but that actually make the check box disappear completely.

    Would appreciate your help. Thanks in advance!
