Public Youtube video Submit

Hi everyone,

I’m looking for a way to allow public users to submit videos that have been posted on YouTube to my wordpress site. Ideally, the form would just consist of boxes for “Name”, “Email Address”, “Video Title” and “YouTube address”, as well as a “Submit” button. I do not want people to have to find the YouTube embed code.
