This sounds like something that is caused by a conflict with another plugin. It would be good to review all 3 orders that this occurred and find out what the common attributes in these orders are. With this info, you can try to recreate the issue. I would advise creating a staging site and recreating the orders that this occurred to and seeing if the same thing happens. You can use sandbox accounts to do this, these accounts should behave in the same way as a live account.
Once you know how to recreate the issue, you can continue to test what plugin might be causing this issue. For this, you may want to perform a full conflict test to rule out eventual issues with the theme or a different plugin. We recommend temporarily activating the default theme Storefront and disabling all other plugins except for WooCommerce and PayPal Payments to see if the behavior persists.

– WooCommerce > Status > Get system report > Copy for support
– WooCommerce > Status > Logs > select “woocommerce-paypal-payments…” in the dropdown and click on view.
