Print icon presents blank pop up in all browsers

hello! i just install Print-O-Matic plugin and i am using it with short code in my page text editor. but when i click on print icon it presents a blank page in pop-up here is the link of my test page. http://imperialsoft.com.pk/tcfusa-oldwebsite/test-page/

i have use short code like this.
