Prefilled Form with user values?

Hi there.
Is there a way of sending a prefilled for to someone and passing in values?

For instance.
I want someone to get in touch with me using a very quick form.
Name, Email
Then I want to direct them to a secondary form to ask them more questions. I would like to pre-fill the name and email address on the second form.

Is this possible?
