Pre-checkout Form

Hello,

I am looking to setup a website running on woocommerce. I would however need to have a multi-question form that needs to be answered before each product can be added to cart. If the user fails to give the “correct” answers, the product would not be added. And if they are registered they only need to answer it once for each product.

I don’t suppose there is any plugin or anything that could help me achieve this?
