Posts show image titles but not images

    After upgrading my multi-site network to WordPress 4.0, we noticed the images on posts on one site is displaying image titles and captions, but not the image itself. Also posts with links to PDFs do not work.

    Attempted fixes included:
    Deactivating all plugins – problem remained.
    Uploading the images again to the post or adding images to a new post.
    Editing the .htaccess file
    Changing the Permalinks back to default.
    Switched themes – same problem with different theme.
    Changed access permissions on image directories and files.
    Upated all themes and plugins to the most recent versions.

    Unfortunately nothing worked out. Please have a look and suggest some solutions.

    If this question has been answered already, please link to the post. When I looked, I didn’t see anything that specifically addressed this problem.
