Hope you are doing well.
Have you tried checking the post editor to see if the excerpt is missing there? Not all themes show the excerpt in the front end by default.
I ran some tests, and it is working fine using the latest version of forminator, if you already checked that please try to reacreate the form.

Also, is there a way to force the submitted posts to all belong to one post category only?
No, I’m afraid there is not any built-in feature that allows you to force user to use a specific category.
