pop up with [objectObject]

    Since I installed Form Maker, I get a pop up with a page not found every time I visit my wordpress page.
    The pop up address is my website address with [object%20Object] appended in the address bar.
    So I had to disable your plugin.
    Are you able to advise on what I should do to fix this?
