One version of this plugin was pushed with an unfortunate bug. We immediately identified the problem and released a new version, and began rectifying the issue with all sites affected.
This is not how Mediavine does business, and we were horrified at the turn of events, and did everything we could to help our publishers fix the issues as quickly as possible. We also made sure everyone had a credit for 3 days worth of ad revenue, even if their site was only down for 5 minutes.
A new testing process has been put in place, and this will never happen again.
