Point Scoring in CF7?

    Hello Guys,

    i really hope to find some adives in here:
    So i have installed the Contact Form 7 Plugin for building really good contact forms and a Plugin (Flamingo) that lets me download received Filled-In forms as .CSV data.

    The problem now is, that my client wants to have something like point scoring in his contact form with checkboxes for each point, like:
    “Please rate the plugin on a scale from 0-10:
    1 [tick]
    2 [no tick]
    3 [no tick]
    4 [no tick]
    ..”

    Is it possible to add something like a rating system in a contact form or do you have any other tips for me?
