[Plugin: WordPress-to-lead for Salesforce CRM] Having CSS issues with WordPress to Lead

I’ve installed according to directions and added shortcode to page here: http://www.businessstrategyconsulting.net/about/

The nice looking form from WP 2.9 is now a huge wide gray background and even with CSS installed its not working.

When I unselect the CSS option, the form still looks the same

Any ideas what the issue is?
