[Plugin: WordPress SEO by Yoast] Using my theme's page Title and Description in Yoast

Hi there,
New to using Yoast so please forgive me if this is a silly question to some of you 🙂
I use a theme called inFocus. I have a 250+ page site and would love to use Yoast however I have encountered an issue.
My site and therefore all my pages and portfolio’s have my themes SEO and for every page and portfolio I have painstakingly filled all these in.
Is there any way I can use, for each page and portfolio my themes TITLE and DESCRIPTION?
Yoast is not filling in the page title or description how I have do so and just wondered if this were possible as there is no way I can redo all 250 pages which would be a shame as Yaost look awesome 🙂
