[Plugin: WordPress SEO by Yoast] focus keywords says NO

WordPress version: 3.4.2
WordPress SEO version:1.2.8.4

I did this:I have all the focus keywords in the title and body of the page but the plugin says…
Your focus keyword was found in:
Article Heading: No
Page title: No
Page URL: No
Content: No
Meta description: No

I expected the plugin to do this: say yes, especiall on the title

Instead it did this: see above.
