I used the latest version of wordpress 3.4.2 and Visitor Maps Extended Referer Field 1.1.9.
In fact i’ve install the plugin on 3 different site with the same issue
i use google chrome and Firefox and IE8 with the same issue

Phil,
If you click the link, “+ Banned IP Addresses”, what happens? It should toggle the visibility of the IP manager.
I ask because I have not been able to reproduce this bug yet. Be sure your browser cache is cleaned.
Are you using touchscreen?

If i press the green bouton, it becomes red and a message tell me this Ip is banned.
See the print screen
And i test the admin with google chrome, Firefox and IE8 with the same issue.

Thanks for the screenshot! I now know what the bug is.
The problem is with the language. This plugin uses javascript to insert the editing tools in proper position. To locate the proper position, the javascript looks for certain words in the source code, such as “Visitor Map Viewer”. I wrote the script to look for English words and not words in other languages.
I will work on a fix for this bug.
I wrote this script for my own site, which is in English. I wasn’t really thinking about the language when I released it to the public. I now see there is a need to make some changes to it.
This is (hopefully) fixed in version 1.2.0.
