plugin to download after filling out a form

    Hello, i am looking for a plugin that allows a user to download documents only after filling out a form.
    I have looked online and did come across a plugin called “email before download” however the download monitor plugin required causes the white screen of death.
    Can anyone recommend a similar plugin?
