There are many form plugins for WordPress. However, you seem to be looking for one that also supports the output of form fields on multiple pages. There is this one, for example: https://wordpress.org/plugins/multi-step-form/
This is also possible with the commercial Pro version of WPForms.
The entries are then usually saved by the form plugin as data records that can be viewed in the backend. You could then create individual evaluations from these, but this is very individual and may have to be programmed. It all depends on what you specifically need here.
yes, i am trying several form maker but i cannot see any that let me create the output. Let’s say it is a kind of self assesment test made with the point that each answer give. The user must be able to have at tend of the test its “profile” Wether i am trying all the free versions (gravity, ninja,wpfors, formidable ecc) i can’t see the possbility to create the end evaluation

Then I would recommend that you simply contact the support of the plugin that is most likely to work for you. They will probably be able to tell you best whether what you are looking for is possible with their plugin.
