[Plugin: SimpleMap Store Locator] Display Search Form

    Having an issue where the form on the actual where to buy page exceeds the template. In other words it stretches outside the window and I know have to scroll left and right on the page. The fields: Street and Zip are fine, but the rest are not on the page. It almost seems the like the input fields are to big. I am currently using a DICE wordpress template.
