    I’m not sure what would cause the labels to be hidden, but not the input fields. Do you have a website where this is currently happening, so I can take a look?

    It’s been disabled, I’m not using the plugin at moment. 😉

    If you do want to try and troubleshoot this in the future, please let me know… I’d be anxious to see what the actual problem is.
