Need help please
1. Have you tried to embed the form in the widget area (sidebar) instead of adding it as a widget in the content area?
2. How exactly do you insert it there, what’s the shortcode value used?
3. What is the data that has to be visible in the frontend, i.e. what’s inserted in the backend?
I would assist further if you send me more details and/or some access later if needed (I noticed the comment on the plugin’s site as well).
Can you answer the other questions then – how you insert it in the page?
Could you try with another theme (like twentyeleven) to see if it works? I don’t see any JS conflicts so there is either an incorrect shortcode used, or no rows, or some theme conflict.

Sorry but my friend bought the theme so I cant use a nonther but if the code above is not right I will try it with a nonther theme but it will not be a solution for me.

The code seems correct, I just need to activate the default theme for a second, refresh the page with the shortcode, see if it works with the other theme and switch back to your theme (it would take seconds).
I would like to help you investigate this, but I can’t do it completely without an access to your site or some assistance and test from you.
