    add_options_page(‘Login LockDown’, ‘Login LockDown’, ‘manage_options’, basename(__FILE__), ‘print_loginlockdownAdminPage’);

    instead

    add_options_page(‘Login LockDown’, ‘Login LockDown’, 0, basename(__FILE__), ‘print_loginlockdownAdminPage’);

    in loginlockdown.php

    The databases table you can create it yourself.

    Its solves your problems this time.

    And to clarify/make it easier to fix:

    In file loginlockdown.php, find line 299:
    add_options_page('Login LockDown', 'Login LockDown', 9, basename(__FILE__), 'print_loginlockdownAdminPage');

    and change it to:
    add_options_page('Login LockDown', 'Login LockDown', 'manage_options', basename(__FILE__), 'print_loginlockdownAdminPage');

    (line number based on the latest public version, v1.5)
