If I want to use the Jetpack contact form, do I have to be connected to WordPress.com? Will it work if my site is hosted by godaddy.com? If so, how do I "connect it?"

All I did was just make a new page and on the text field next to the images thing you will see the contact form thing and input that into your new page.

but first I think you have to configure it from the beginning like all the programs in jetpack

Yes, you have to connect to WordPress.com and Jetpack will work with GoDaddy web sites. When you activate Jetpack, you should see a banner at the top of your plugins page with a "Connect" button in it. If you do not see this, change your theme to Twenty Eleven temporarily and it should show up.
