[Plugin Jazzy Form] unable to find form

    i have installed Jazzy Form but unable to find out form where it is placed so that i can make changes or can make form according to my requirements.According to its FAQs ” This software adds a “Forms” item to WordPress’es menu in the sidebar. That’s where you set up your forms.
    Can any body help..!!
