[Plugin: Formidable Forms] Form creator not working

I had a site crash and had to re-instal the back up.
Once I was up agian I found that my formidable forms disapeared (seems it was not backedup)
I tried to re-install the Formadible plugin but it does not workproperly. My other website works perfectly and the plugin came with a contact templet. This one not… please advise
