plugin form, need help

Need some help to find a form, I used Contact Form 7 but the client wanted it to be set up in a differrent way, so I need something that can do the following:

After they fill out how many people that will attend a trip, new rows will show up where they can fill out name and age of the people going.
So if they write 3 people then 3 text fields will show.

Any suggestions
