These settings are probably incorrect:
Check the WP settings in Admin – Settings – General
WordPress address (URL)
Blog address (URL)
I had changed both of these settings to http://www.fuddster.com. I’m currently looking into other things I may have overlooked.
This page is 404 so something is wrong.
The plugin folder may have the wrong permissions (try CHMOD si-contact-form to 755)

I just double checked and all folders and sub-folders were already set to 755.
Seems as though some part of my WP installation didn’t recognize the move out of the sub-folder to the root.
I recreated the sub-folder and copied the si-contact-form to its previous location and the test worked:

I wound up reinstalling my site. Everything’s working fine now. Thanks for the help.

i am having the same problem that i cannot see the captcha image on my site. i tried disabling it for the time being and while it dissapeared on firefox, i can still see the broken captcha image on safari. i checked the plugin folder and it is set to 755. i have turned the captcha back on. can you please help? brandonsparks.com

when i test to see if my php sill support captcha, it fails the test.
404? check folder permissions for the si-contact-form and all is subfolders (should probably be CHMOD 755)

Then are files missing? because I get 404 not found on the captcha test. maybe you have a incomplete installation or your permalinks are messed up. Temporarily try the “Default” permalinks. Admin – Settings – Permalinks.
still doesn’t work. is there a way i can completely uninstall all of the files in the plugin and then do a reinstall?

i uninstalled the plugin, emptied my cache and reinstalled. works great!
