 * How can one add options with check boxes? I need to require people to accept 
   terms and conditions but this plugin does not allow it.

 * You can just select the radio field and limit it down to option. Just title it“
   Agree with terms” It doesn’t auto fill it by default, so it will make your customers
   check it if you need them to agree.
