 * how can i remove the “website” form and title I tried un-checking the radial 
   under guestbook settings > extended but it is still there?

 * You removed only the website value on every post. If you like to remove that 
   in the form, edit default.tpl on “../plugins/dmsguestbook/template/form/” with
   an editor.
