[Plugin: Custom Contact Forms] CVS export throws white page on WPMU

Hi Taylor, first of all congrats on a job well done. The plugin allowed me to put together a customized contest form that will soon go live. Love the flexibility.

Reason I chose your plugin to work for the purpose of a giveaway is that we want to collect and export the submissions. I tried to export a few test submissions but only encountered a white page instead of a CSV file.

When attempting to either download all saved form submissions or only for a particular form ID, I get blank pages. The URL for the white page is

http://domain.com/wp-content/plugins/custom-contact-forms/download.php?location=export/

I run WP MU 3.3.1 and except for this problem the plugin works flawlessly. PHP v5.2.17
