[Plugin: Custom Contact Forms] Attached Fields not showing

    Hi,

    I just installed ver 4.0.9.2 and having problems with setup.

    I created a form and attached fields. Then I made a contact us page and just used the [customcontact form=2] code in the post.

    Everything seemed fine until I started to stylize the form. I created a new style and applied it to the form. Now only some of the fields are displayed on the contact us page. No major changes to new style from the default – just increased text & textarea field widths.

    So not sure why some of the attached fields don’t appear on the contact page?
