[Plugin: Contact Form 7 Dynamic Text Extension] Both $_POST and custom field values are not showing

Not sure what I have done wrong. I activated the plugin and edit a CF7 form as per your installation instructions:

Just the short codes never convert to show me the field values:

What have I missed?
