[Plugin: Contact Form 7] Double Contact Form?

If you’ll look at http://www.becomingtheiceman.com/contact

You’ll see that there are two contact forms… Yet I don’t have two contact codes posted. Ever since I updated, this has been a problem an issue.

Here is my code:

(I only have the form listed once in the actual page)
